Caesarean scar: care, massage and when to worry

A line that asks for a little gentleness and a lot of patience

By Laetitia 7 min read

A caesarean scar closes in one to two weeks and becomes strong in about six weeks, but it needs twelve to eighteen months to fade and soften. A gentle shower every day, patting it dry, then massage and sun protection once it has closed: these are the key steps.

You barely look at it in the first days, then you gradually get used to this line, just above the pubic bone. It pulls, it itches, it changes colour, and you often wonder whether that is normal. Here is how it changes, how to care for it, and the signs that mean you should seek advice.

Key points

  • A daily shower as soon as the team allows it, mild soap, pat dry.
  • Itching, pulling and numb skin around it are common and temporary.
  • Massage starts once the scar has fully closed, often between the fourth and sixth week, with your midwife's agreement.
  • No direct sun on the scar for a year.
  • Spreading redness, heat, weeping, fever or a scar that opens: seek advice quickly.

How long does a caesarean scar take to heal?

The skin closes in one to two weeks. By about six weeks, the deeper tissues are strong enough for most activities. The scar then keeps remodelling for twelve to eighteen months: pink or red and slightly thick at first, it gradually becomes flat, supple and pale. This slow pace is normal.

PeriodUsual appearanceWhat to do
Days 1 to 5Dressing, thin line, slightly swollen edgesLet the team keep an eye on it; shower as soon as they allow
Days 5 to 10Stitches or staples removed if not dissolvable, small scabsShower, pat dry, loose clothes
Weeks 2 to 6Scar closed, pink or red, itching and pullingDo not scratch, keep watching, start massage once it is fully closed
Months 2 to 6Redness fading, ridge flattening, area sometimes numbDaily massage, sun protection
Months 6 to 18Paler, more supple lineSun protection for at least a year, advice if it thickens

How do you care for the scar in the first days?

In hospital, the team checks the wound and decides when the dressing comes off, often after one to two days. After that, a daily shower is enough: let the water run over it, wash with a mild, fragrance-free soap, rinse well, then pat dry with a clean towel. No antiseptic or cream on the wound without medical advice.

  • Loose clothes and knickers whose elastic sits above or below the scar, without rubbing.
  • A clean towel for your tummy, changed often in the first days.
  • No bath or swimming until the scar has fully closed: ask your midwife when.
  • Stitches or staples that do not dissolve are usually removed between the fifth and tenth day, in hospital or by a midwife at home.

To cough, laugh or get up, holding a pillow against your tummy helps. The rest of recovery is covered in our guide to recovering after a caesarean.

Why does a caesarean scar itch, burn or pull?

Itching is part of healing: the skin rebuilds and small nerve endings repair. Burning or tingling, and pulling when you sit up, are common in the first weeks and ease afterwards. Do not scratch: tap gently around it, wear cotton and, once the scar has closed, moisturise the skin of your tummy.

Pain that gets worse instead of better, or itching with spreading redness, is not just a stage: it should be checked.

Why is the skin around the scar numb?

To reach the womb, the incision goes through the skin and small sensory nerves. The area just above the scar therefore often stays numb, almost like cardboard, for several months. Feeling usually comes back gradually, sometimes not completely. It is not dangerous. Massage, once the scar has closed, often helps you reconnect with the area.

When and how should you massage a caesarean scar?

You massage a scar that has fully closed, with no scab or stitch, often from the fourth to the sixth week, after talking it over with your midwife or doctor. A few minutes a day is enough. Massage softens the scar, stops it sticking to the tissues underneath and helps you get used to this part of your tummy again. It should not hurt.

  1. Wash your hands and lie down, knees bent, tummy relaxed.
  2. Put a little neutral oil or balm on your fingertips so they glide without pulling.
  3. Make small circles along the scar, then on either side, pressing gently.
  4. When the area is less sensitive, gently pinch the skin between thumb and index finger and roll it along the scar.
  5. Stop if the scar turns red, hurts or bleeds, and ask for advice.

A midwife or physiotherapist can show you how, for example during your pelvic floor rehabilitation.

Which cream or oil should you put on a caesarean scar?

Until the scar has closed, nothing other than what the team prescribed. Once it has closed, a neutral, fragrance-free oil or balm is enough for massage: no product has been shown to erase a scar. For a scar that thickens, a doctor may suggest silicone sheets or gel. Ask your pharmacist or midwife for advice.

Is the overhang above the scar normal?

Yes, it is very common. Just above the scar, the tissues can form a slight fold, made more visible by swelling in the first months and skin stretched by pregnancy. It often softens a lot within six to twelve months, with time, massage and a gradual return to activity. It says nothing about your efforts: it is how the abdominal wall heals.

Should you protect the scar from the sun?

Yes. A recent scar darkens easily in the sun and can stay darker than the surrounding skin. For at least a year, cover it with clothing or a very high factor sunscreen when you are in the sun. As it sits low, high-waisted swimwear is often enough to protect it on the beach.

A thick, red or growing scar: what is it?

Sometimes the scar stays red, thick and raised beyond a few months: this is called a hypertrophic scar. More rarely, it spreads beyond the incision line and keeps growing: this is a keloid, more common on darker skin and if you have had one before. In both cases, talk to your doctor: treatments exist.

When should you worry about a caesarean scar?

When to seek advice

Call the maternity unit, your midwife or your doctor quickly if:

  • redness spreads around the scar, the area is hot or swollen;
  • the scar weeps, lets out pus or smells bad;
  • the edges pull apart or the scar opens, even slightly;
  • the pain gets worse instead of better;
  • you have a fever of 38 °C or more, or chills;
  • a hard, painful lump appears under the scar;
  • later on, the scar becomes painful or swells with each period.

Heavy bleeding from the scar or feeling faint: call 15 or 112 in France (999 in the UK).

Frequently asked questions

When can you shower after a caesarean?

Often from the next day or the day after, depending on the maternity unit. Let the water run over the scar without rubbing, then pat dry.

When is a caesarean dressing removed?

The team usually removes it after one to two days, sometimes later depending on the type of dressing. After that, the scar is usually left open to the air, protected by loose clothes.

Can a caesarean scar reopen?

It is rare. It mainly happens in the first days, often because of an infection. If the edges pull apart, or the scar weeps or bleeds, seek advice quickly.

How many layers are stitched during a caesarean?

Several: the womb, the sheath covering the tummy muscles, the fat under the skin and the skin. The abdominal muscles themselves are moved apart, not cut.

When can you put cream on a caesarean scar?

Once it has fully closed, with no scab or stitch, often after a few weeks. Before that, use nothing other than what was prescribed.

Why does my caesarean scar hurt months later?

Pulling or brief twinges are possible for months, while the scar remodels. Persistent pain, or pain that returns with every cycle, is worth a consultation.

Does a caesarean scar eventually disappear?

It does not disappear, but it usually becomes a thin, pale line, hidden below the bikini line, within twelve to eighteen months.

A line that slowly fades

This scar tells the story of a birth. It asks for little: gentleness, a bit of patience and a few minutes of massage, when the time comes. If something worries you, you can always show it to someone. For an overview of the weeks after the operation, read our guide to recovering after a caesarean.
